Caseworkers warned of children being at risk before four-year-old boy died 07.07.2026

A four-year-old boy has died in Wyong, Central Coast, following an alleged murder by his 32-year-old mother. The tragedy has sparked intense criticism regarding the Department of Communities and Justice (DCJ) and a failing child protection regime. Caseworkers warned that children were at risk due to systemic gaps caused by the transition to "Families Together," a new umbrella framework for family preservation. While the government claims all families on the coast have been transitioned, caseworkers report being unable to refer at-risk families since January 2026 due to closed legacy programs and confusion over the new system. This follows damning Ombudsman reports stating that many "risk of serious harm" reports are closed without in-person assessments due to lack of capacity. The death highlights critical concerns that vulnerable families are falling through gaps during this massive structural overhaul.
