Introduced in 2020 to address a nursing shortage during the COVID-19 pandemic, Germany's "Pflegebudget" allows hospitals to directly bill statutory health insurers for nursing personnel costs without extensive review. While this policy has led to a significant increase in full-time nursing staff, rising by 18% to over 350,600 between 2019 and 2024, it has coincided with a 12% decrease in patient cases. Consequently, nursing personnel costs for statutory health insurers have surged from €19.4 billion in 2020 to €26.1 billion in 2024, with costs increasing by 10.5% in 2024 alone. Despite the increased spending, the adherence to minimum staffing levels has not improved, and concerns are mounting about potential misuse of funds, leading to proposed legislative changes to restrict eligible expenses and a call for a fundamental reevaluation of the system.
